1. Describe the issue or challenge that your college’s practice was designed to address.

2. Describe the process, timeline, participants and resources required to implement the practice.

3. Identify positive outcomes (cost savings, revenue/resource enhancement, etc.).

4. Elaborate on lessons learned and ease of transferability, so that other colleges may benefit from your practice and experience.

Limit your Practice Summary information to no more than 3 double-spaced pages.
Do not include additional information (reports, forms, etc.) with the application.
Program judges may request additional information based on your application during the evaluation process.
